First Visit
Our team wants your first visit to be the best dental experience you've ever had. We appreciate you choosing us for your dental care. We will consult together about what is most important to you now and for your future dental health. Once we get to know you, we'll discuss any issues or concerns you may have and create a custom plan for your dental health.
What to bring
We’d love for you to bring a few things along for your first visit.
- Photo ID (such as a valid driver’s license)
- New Patient Forms and Insurance card or Full Policy Information (for those with dental insurance)
- Questions, concerns, and hopes for your smile
- A list of all current medications
finances & Insurance
We offer a variety of payment options to work within your budget including partnerships with CareCredit and Proceed Finance. We will also be happy to file claims with your insurance.
Quality still lets you use benefits. Our team will maximize the benefits in your policy towards treatment and routine care. As an unrestricted provider for insurance companies, we will complete and file your claim to your insurance carrier.
Membership Plan
Patients who choose to become members at our office enjoy the following benefits:
- Professional dental cleanings (2 per year)
- Doctor exams (1-2 per year)
- Needed x-rays
- One emergency exam per year
- 10% off all other treatments
Starbody Dental is committed to giving you specialized care at a reasonable fee. We will make every effort to estimate your costs before your visit and payment is due at the time service is rendered. You are responsible for all fees not covered by insurance. If additional tests are needed such as lab work or a biopsy, you will be billed directly by those providers and will be responsible for submitting payment directly to them. Your prompt payment is appreciated. Payment arrangements may be considered if discussed in advance of treatment.
